Please complete our Libqual Survey for a chance to win a Fitbit Charge HR!
Dear Library User,
To help us plan improvements here at the Ruth Lilly Medical Library, we are participating in a nationwide project to measure the effectiveness of library services and resources. The assessment is being done with a web-based survey, using the LibQUAL+™ instrument. The LibQUAL+™ survey is designed to assist libraries in soliciting, tracking, understanding, and acting upon users opinions of service quality. I am hoping that you will be able to contribute a few minutes of your time to help us better measure how well we serve the IU School of Medicine community. Please focus your answers on the Ruth Lilly Medical Library even if you use University Library or Law Library more frequently.
Your response to the survey is anonymous. However, one Fitbit Charge HR will be awarded to one randomly selected person in each of the following categories:
• IUSM graduate medical student (resident, fellow, etc)
• IUSM undergraduate non-medicine student
• IUSM graduate non-medical student
• IUSM faculty
• IUSM staff
If you wish to participate in the random drawing, please provide your IU e-mail address at the end of the survey. E-mail addresses are NOT linked with responses but are needed if you want to be eligible for the drawing for the Fitbit Charge HR. You must click the "Complete the Survey" button at the end of the survey for the survey to register as complete.
If you wish to participate, click on the URL below and on through to the survey.
The survey will run until November 24th and we will contact prize winners via email. Participants at IUSM campuses other than Indianapolis will have their prizes mailed to them.
Thank you for your time and providing suggestions for improvements.
